When I was a kid 40+ years ago I remember a tag like that hanging from the rear view mirror of the cars that were getting service done at the Phillips 66 station, on the tag it said what kind of oil you wanted, greese , air water , kind of like a check list, plus your name and address,I think it said FILL ER UP with PHILLIPS GAS and OIL in red letters at the top of the tag I don't know if thats what you have here , but it could be some form of that.
